CONFIDENTIALITY STATEMENT

Serenity Recovery Centers operates under the Federal Confidentiality guidelines (Public Health Service Act, title 42 section 290dd-3) and Privacy Regulations.  These regulations restrict the disclosure and use of patient identifying information about individuals in substance abuse treatment.

I agree and understand that as a volunteer of Serenity Recovery Centers, that any and all information that I may see, hear or read while on the Serenity Recovery Centers property will remain confidential to this facility.
